Multifaceted home based psychiatric healthcare services are provided, under the direction of Yellowbrick Medical & Professional Staff, in the privacy of your home or apartment.
A strengths-based neurobiological approach to helping the emerging adult discover, develop, and utilize their personal strengths and innate competencies as they work towards an independent, productive and meaningful life.
Yellowbrick provides Core Competence Home Health for emerging adults ages 18 – 35. Yellowbrick provides in-home support services to emerging adults living in geographic proximity to Evanston, adjacent to the city on Chicago’s North Shore. Yellowbrick Professional Staff provides comprehensive home health care, including occupational therapy, registered dietician, nursing and medication support services, and social work counseling. Core Competence Staff (CCS) provide support at home and in the community to accompany the emerging adult as they pursue challenging activities. For example, this may include grocery shopping for an individual struggling with an eating disorder or getting to school or work on time for someone who has difficulty with ADHD, self-organization or self-motivation. The specific focus, tasks and frequency of Core Competence Home Health are mutually negotiated and individualized.
Core Competence Home Health meets the diverse needs among varied groups of emerging adults. It is an optimal choice for many emerging adults who find that Core Competence Home Health allows them to safely and effectively sustain themselves, both personally and academically. Emerging adults with autism-spectrum disorders, or significant non-verbal learning disabilities are often able to achieve a higher level of independent functioning with Core Competence Home Health.
Core Competence Home Health provides a mutually agreed upon level of staff availability, membership within a community of peers and services which support life skills, interpersonal connections and educational or career goals. Each emerging adult works with an Advocate who coordinates planning and communication while providing coaching for problem solving and skill development, as well as an education and career specialist who addresses work and academic needs.
Family education and support are integrated into the treatment plan.
Emerging adults within Core Competence Home Health are also eligible for participation in the many recreational and creative therapeutic activities within Yellowbrick such as the art studio, health club membership, movie discussion night, community dinner and brunch, Chicagoland cultural adventures and “wilderness” outings.
Yellowbrick has access to information on rental residences in Evanston and Chicago that are proximate to our offices in downtown Evanston, and are comfortable and safe with access in their neighborhood to many amenities. Our staff is available to discuss these alternatives with you. Yellowbrick has direct communications with the following services that will assist our residents to find appropriate housing close to our professional offices.
