Yellowbrick Self Assessment
  1. Has your loved one lost interest in favorite activities?

  2. Does your loved one exhibit signs of extreme sadness and extreme happiness?

  3. Do they sleep more than normal or not at all?

  4. Have they recently gained or lost weight?

  5. Do they often appear irritable or angry?

  6. Do they exhibit signs of self-abuse (cuts, bruises)?

  7. Are their grades starting to slip or are they having problems at work?

  8. Have they severely limited their activity? For example, do they stay inside and avoid people or places they once enjoyed?

  9. Do they drink in excess?

  10. Do they prefer to drink alone, rather than with others?

  11. Are they using illegal substances?

  12. Are they preoccupied with food?

  13. Do they diet excessively and/or abuse laxatives, diet pills or diuretics?

  14. Have they become isolated from family and friends?

  15. Have they ever mentioned harming themselves or others? If the answer to this question is yes, please seek help immediately by calling 911.
